Visiting Us for the First Time

We are currently accepting new patients and in-network with many insurance plans. For your first appointment, please bring your insurance card, driver's license or ID, and your completed registration forms. Please call (972) 966-1079 with questions about our practice, your insurance, or to schedule an appointment.

Medical Records

For us to provide the best care for our patients and future patients and accordance with state laws, medical records can take up to 15 business days to be processed and may incur a $25 fee unless considered continuation of care.

When would I need to request medical records?

  • If you or your child is a new patient, you are encouraged to bring the medical records pertaining to the reason for the visit in your first appointment. This may include records from your primary care physician and/or specialists. This is not required, but discussing pertinent items in your history with our providers can be helpful in your treatment.
  • If you are an established patient wanting to request records to be sent to HV Mental Health & Wellness for treatment or progress updates.
  • If you are an established patient needing records sent to another entity such as another provider’s office, an attorney, or for personal record keeping (Please note that psychological test data is not part of a patient’s record, and therefore requests for raw data will only be released to another mental health professional.)
